I can't get my guest book to work

so artists can put their corrections and comment on the page with the video it should be simple code but can get it to post any comments

any ideas?

I do not see any Guestbook, where are you trying to use it?
I went to your page and im not sure where your trying to integrate it into.

Deleted it going with Jamoola instead more complete solution for my sites all round.
